First Visit

For your convenience, you can download, print and fill out the following forms prior to your first visit. If applicable, mail the Record Release Form to your previous physician's office to request that your child's records be sent to our office. If you wish to transfer your records to another office, you may also use the Record Release Form.

After completing the forms, please bring the following with you to your appointment: New Patient Registration, Medical History Sheet, HIPAA Acknowledgement Letter and the Financial Policy.
